HELPFUL INFORMATION

PRESCRIPTION FOR TREATMENT

Most patients come to us with a prescription for physical therapy that has been issued by their physician and includes specific treatment recommendations. Some insurance plans will not pay for physical therapy without a written PT referral, while others will. If you have any questions regarding your specific insurance requirements, please contact your insurance provider.

PAYMENT INFORMATION

We are happy to verify your insurance benefits and confirm our participation in your specific insurance policy before your first appointment. Please have your insurance card ready when you call to make your appointment. This will enable us to confirm your benefits before your first appointment.

Our cash rate is: $200 EVAL & $125 follow-up.

WHAT TO WEAR

We recommend that patients wear loose-fitting attire or exercise clothing and sneakers for maximum comfort.

Physical Therapy prescription/referral from a physician
Picture ID
Medication List
Reports from any diagnostic tests (X-rays, MRI, EMG, etc.)
Operative reports
Any braces, prosthesis, or assistive devices you are using

WHERE TO PARK

LA CAÑADA: Aspire Physical Therapy and Sports Performance offers free parking under the office building; street parking is also available. We are located upstairs on the third floor. For your convenience, an elevator is located in the center of the Plaza, as well as several staircases that lead to the third floor.

SAN FERANDO RD/GLENDALE: You are free to park in any of the spots within the Elevate Fitness Complex.

PASADENA: ASPIRE is pleased to provide all visitors with 75-minute parking validation. Any time spent exceeding the allotted 75 minutes will be subject to the following rates, as outlined in the attached document. Please use the valet services; no tips are required.

PATIENT REGISTRATION FORMS

Please arrive 10-15 minutes early to allow time to complete your new patient registration forms, or take a few moments now to review and print the forms. Upon arrival at our facility, our staff will greet you and review the necessary forms with you. We will schedule all of your appointments and collect any payments (if applicable) at each visit.

MEETING YOUR THERAPIST

You will meet with a Doctor of Physical Therapy (DPT) who will evaluate your injury or condition, discuss your goals and specific needs, and develop an individualized treatment plan.

NEXT STEPS

The length of your treatment plan and frequency of visits are based on your specific injury/condition, the physician’s prescription, your insurance plan, and the DPT’s recommended treatment plan.

HOME EXERCISE PROGRAMS

You may be provided with a home exercise program that complements your care plan. The home program is a crucial component of the healing process and will help you achieve your goals.
